• Land Development Application (DOC or PDF)
• $650 fee check
• Fee Calculation Sheet
• Boundary Line Adjustment Checklist, signed by surveyor or engineer
• 5 plats (folded) with original signatures of property owners and seal and signature of engineer or surveyor
• Original stamp and signature on plat from Health Department
