Major Site Plan Amendment Submission Requirements (no additional acreage or additional units):

• Land Development Application
  (DOC or PDF)
• $1,000 fee check
• Major Site Plan Amendment Checklist
• Erosion and Sediment Control Checklist (if applicable)
• 10 copies of plan, folded
