Inspection Forms Submission Requirements:

  1. Download the proper form based on the type of stormwater management/BMP facility that you are inspecting.  A separate form is required for each stormwater management/BMP facility that is maintained by the owner or responsible party.
  2. Complete the digital form or print legibly.  Please complete the entire form and SIGN the bottom of the form.
  3. Take two photos of each facility being inspected.  One photo should include the facility with the house or building in the background.
  4. Mail the completed forms and photos to the VSMP Administrator at:

      5. Repeat this process for each facility that is maintained
          by the owner or responsible party.

      6. No fees are required.

** Any underground system with manhole access that requires underground access in order to complete the inspection must be inspected by an OSHA-certified confined space professional or a DEQ certified stormwater management inspector.
