Fauquier County and Fauquier County Public Schools is committed to providing you and your family with affordable, substantial health benefits under the Affordable Care Act (ACA). All employees who are eligible for health insurance during the 2016 calendar year will receive a tax Form 1095-C. The form identifies:
- employee information
- employer information
- months during the year the employee was eligible for coverage
- the name of the individuals covered under the employee’s policy
The law requires that employers provide a copy of the Form 1095-C to eligible employees by March 2, 2017. A sample of the form is displayed below.
Your 1095-C form will be accessible by logging into your ePortal account. Once logged in, click Pay/Tax Information then 1095-C. For retired and Cobra participants your form will be mailed to your home address on file.
