Starting on November 11, 2019, York will be branded as Sedgwick. This does not change anything with your current leave of absence and you can expect the same service you are receiving today, just under a new name and brand – Sedgwick. You don’t need to do anything as a result of this brand change. It is important to note that York will remain a legal entity at this time, so you will continue to see the York legal name on select correspondence.


What will not change:

  • Your existing York program materials and wallet card will not need to be updated as a result of the rebrand. All contact information is still accurate. Updated materials will be distributed as soon as possible.
  • Existing phone numbers will stay the same.
  • At this time, you will continue to utilize as needed.


What you can expect to change:

  • The Sedgwick name and logo will be on all letters and other communications.
  • Our email address will be “” Emails sent to our current email address(es) will be forwarded; however, will not change as noted above.
  • The URL to our employee portal ( will be If you attempt to access the previous URL, you will simply be redirected to the new site.
  • Our website ( will direct you to and will continue to provide you with access to portals and opportunities to engage.


 Fauquier County Government and Fauquier County Public Schools has a partnership with Sedgwick to administer the Family Medical Leave Act (FMLA) program. 

To request a Leave of Absence

  1. Notify your supervisor of the need for the absence.
  2. Call Sedgwick at 888.436.9530 or visit to initiate a request for leave.
  3. Provide the information requested to Sedgwick.

Once you request a claim, you’ll receive a packet of information to be completed and returned to Sedgwick by the specified time. A Sedgwick claim representative will evaluate your request once all documentation has been received.

Your claim rep. will maintain contact with your throughout the process and can answer any questions you may have about your leave of absence. Your claim rep. will also follow up with the Human Resources representative to ensure timely and accurate processing of your claim. 


Am I eligible for FMLA leave?

You're eligible if you meet the following requirements:

You've worked as least 12 months for your employer

You've worked as least 1250 hours in the 12 months before you requested leave

You work at a site that has at least 50 employees within 75 miles

What circumstances does FMLA cover?

The following leave reasons are covered:

Your own serious health condition prevents you from being able to perform your job

You need time off to care for your spouse, child or parent with a serious health condition

Your child is born or placed with you for adoption or foster care

You need time off for a qualifying exigency, when your spouse, child or parent is on active duty on the Armed Forces

You need to care for a covered service member who is your spouse, child, parent or next of kin (up to 26 weeks of leave may be taken for this purpose)

How much notice should I give my employer and Sedgwick?

It depends on the circumstance:

If your need for FMLA leave is foreseeable(like a pregnancy or scheduled surgery), then notify your employer and Sedgwick at least 30 days ahead of time.

If your need for FMLA leave is unforeseeable, then notify your employer and Sedgwick as soon as possible once you become aware of the need for leave.

What information is needed to support my FMLA claim?

The most common documentation required is a medical certification completed by a health care provider.

Sedgwick will notify you of all needed documentation and deadlines.

What if I'm not sure whether FMLA would apply to my need for time off?

You'll want to contact Sedgwick to request FMLA leave if one of the following situations applies to you:

You've been absent three or more consecutive days for medical reasons, and/or to care for a family member who is sick

You have multiple absences due to the same health condition

You have multiple doctor visits and/ or appointments for treatment

You've added a child to your family

You have absences related to a spouse's, child's deployment to active duty
